Preparing for the transition from school to adulthood for youth with disabilities and special healthcare needs can be challenging and scary for families. It can be difficult to know where to begin or what the priorities should be.
Who should apply:
Parents of transition aged students (ages 13-14) who have an IEP or 504 plan. Participants must commit to attending both sessions.
