The Rooted Hearts Autism Toolbox

Thank you for your interest in receiving a Rooted Hearts Autism Sensory Toolbox by Rooted Hearts Ministries Inc. This service is being provided for families of children 0-8 years old who have been diagnosed with autism spectrum disorders (ASD) in the last 12 months and/or have recently had a disruption of therapy.  If you are deemed eligible, your child will receive one (1) sensory box at no cost, courtesy of our generous donors. All boxes are mailed monthly; please expect an email to the email address provided once your toolbox has been shipped.  We are currently fulfilling requests for the United States and Puerto Rico only due to funding. If you are interested but reside outside of the U.S., please complete this form and a member of our team will be in touch. While we will do our best to fulfill your request, priority is currently placed on families with children 0-8 years old with a diagnosis of autism spectrum disorder in the past 12 months.
