The Children with Special Health Care Needs Program (CSHCN) is a joint federal/state and local program; which has been in operation in one form or another since 1937.Talbot County Health Department’s CSHCN program accepts children from birth to age 22 with a disability; chronic illness or health related educational and/or behavioral problem. Eligible diagnoses cover a wide range from asthma and learning disabilities to autism; cerebral palsy; genetic syndromes and many more.Two part-time staff provide a variety of services for children and parents; including but not limited to initial consultations; home visiting; resource and referral; advocacy; support and education groups; case management; a quarterly newsletter; financial assistance for specific prescribed items (not covered by another source) and other gray area situations.
