An individual (or other interested party); may either walk in to the local Health Department OR call in advance to schedule an appointment with a coordinator to discuss service requestsIndividuals Have the Following Types of Disabilities:Deafness/Severe Hearing Impairment; Orthopedic Impairment; Autism; Behavioral Problems; Blindness/Severe Visual Impairment; Cerebral Palsy; Cystic Fibrosis; Epilepsy/Seizure Disorder; Head Injury; intellectual disability; Multiple Sclerosis; Muscular Dystrophy; Speech/Language Impairment; Spina Bifida; Spinal Cord Injury; Other Neurological Impairment.To apply for Developmental Disabilities Administration (DDA) funded services; an individual will need to complete a DDA Application for Services form; and a Critical Needs Recommendation form. They will also need to submit with their application some documentation of their disability. These forms may be obtained on-line or in person from any Resource Coordinator.
