Coordination of Community Services provides information/referral and case management services for children and adults with developmental disabilities and their families. This program:

• Assists families in locating and arranging diagnostic evaluations, treatment, day programs, residential support and individuals/family support services.
• Provides, for individuals in service, assistance in implementing individual choice addressing individual satisfaction and assuring that the individual’s needs and preferences are addressed through the Individual Plan process.
• Assists the individual in identifying, locating and brokering services, which may include generic programs, natural supports, other state programs including Medicaid State Plan Services and services funded by Developmental Disabilities Administration (DDA).

The Family Support Network (FSN) helps to meet the needs of families with children; birth to 3; with developmental delays or disabilities in Maryland. Each local FSN provides information to families regarding community services; refers families to local support groups; workshops; and advocacy groups; provides opportunities for families to network and share ideas and experiences; and helps to link ?experienced? parents to parents of newly identified children with similar special needs.

OUR MISSION: Maximize the health of citizens by providing access to opportunities that promote physical; behavioral; and environmental health through preventive; educational; assessment; clinical; monitoring and regulatory programs.

Infant & Toddler ProgramThe Wicomico County Infants and Toddlers Program provides an array of services to children from birth through age three. The Program is family-centered & community based. Services are designed to maximize the developmental potential of children while providing support to families. Through collaboration with hospitals & community agencies; the Program also helps families navigate the range of services that are outside the scope of the early intervention system. Services are provided by the Wicomico County Public Schools; the Wicomico County Department of Health and the Wicomico County Department of Social Services. At the age of three; if the child continues to be eligible for services; the family may choose to continue on an Extended IFSP until the age of four. Services will remain in the natural environment. At the age of four; if the child continues to be eligible; an individualized education plan (IEP) will be written. Services are provided at no cost to families.
